Could you please let me know the procedure for Transfer of Ownership

Could you please let me know the procedure for Transfer of Ownership "After death of Registered Owner"?
Legal heirship is in the Name of the deceased person's widow and two children.
Both the Children are Major.
The ownership need to be transferred to the name of the Widow.

The documents she has are,

1. Death certificate.
2. Legal heirship certificate (In the Name of the deceased person's widow and two children)
3. Form 31
4. RC book and Tax receipt.
5. Insurance (Expired)

Affidavit needs to be produced as well, stating that the Children have no objection.
But don't have any information on how the Affidavit is prepared. Does it need a stamp paper? Or normal white paper is enough? Revenue stamp is to be pasted on the Affidavit? And what should be the content of the Affidavit?

Also, any other document needs to be produced apart from the ones I have mentioned above?
